Declan Rice has been managing so-called 'neurogenic' hamstring pain for some time at the 2026 World Cup. The England midfielder was substituted in the second half of the win over Croatia after feeling discomfort high in the hamstring and towards the lower back. According to the national coach, it was a precautionary measure.
Rice explained afterwards that he has been dealing with this specific pain stimulus since the post-Christmas period, a complaint that he also kept under control at Arsenal in the second half of the season. “I felt a bit of that neurogenic pain again,” he said in the mixed zone. At the same time, he emphasized that he is fit and can play against Ghana on Tuesday.
Neurogenic pain differs significantly from a classic hamstring injury, where there is actual damage to muscle fibers or tendon tissue. In this case, it concerns stimulation of the sciatic nerve, which runs from the lower back via the pelvis to the back of the leg. Irritation in this area can cause hamstring-like pain without the presence of a muscle tear.
This stimulation can affect the so-called posterior chain — glutes, hamstrings and calves — which are essential for acceleration, sprints, dueling power and intensive pressing. The body can unconsciously 'protect' an irritated nerve, causing force production and movement patterns to change slightly. That is why the load is carefully dosed, even if there is no crack.
The physical load for Rice this season is enormous. If England reaches the final and he plays every match, he can end the year with around seventy appearances. Partly for this reason, the medical staff remains alert to signals and is evaluated on a day-to-day basis.
From a sporting point of view, Rice can hardly be replaced one-on-one. He shields the centre-backs, carries the ball forward under pressure and is responsible for an important part of the standard situations. It is therefore crucial for England that his complaints remain manageable during the remainder of the tournament.
Rice is expected to be available against Ghana. In the meantime, the staff continues to monitor and manage recovery times, so that the midfielder can continue to fulfill his key role without unnecessary risks.
